How do I get a copy of a vital record?

If you are not sure if the record you are looking for was recorded in Beverly, please call before placing an online or mail order.

To pick up in-person:

Please come to the City Clerk's Office at 191 Cabot St. during regular business hours. The cost is $10.00 per certificate. Bringing a photo ID is recommended as it can be required for some records.

To request online:

Please visit the Unipay website for online payments. Once online orders are processed, the requested certificate(s) will be mailed to you.

To request by mail:

Please print the request form (below) and mail it with the fee of $10.00 per certificate, payable to The City Of Beverly. Please include a self-addressed envelope. Once the application has been processed, the requested certificate(s) will be mailed back to you in the provided envelope. Request forms with payment and a return envelope should be mailed to the City Clerk’s Office, 191 Cabot Street, Beverly, MA 01915.
